Beginning Services

 
 

To begin services with Heritage the Physician, Nurse, Assistant, or interested party needs only to call Heritage at:

The Heritage nurse reports to the office or the patient's home within 2 hours, or when requested by the physician. At that time the nurse:
  1. Meets with the office nurse, or social worker, physician, or patient
  2. Reviews the patient's chart
  3. Gathers all referral information
  4. Meets with the patient and the patients family
  5. Answers any questions about home care
  6. Evaluates for needed services: Nursing, Home Health Aide, Therapy, Social Services, Extended Hours, Emergency Response System, Medication Dispenser
  7. Contacts Doctor for orders
  8. Orders Durable Medical Equipment if necessary

This saves time for the physician, hospital, or nursing home. It also enables the patient and the nurse to become acquainted with each other. The Heritage Nurse will also serve the Physician regarding home care at case conferences, at family conferences, or over the phone for questions.
Heritage accepts Medicare, Medicaid, Passport, Waiver, Insurance, and Self Pay.
