o        Eligible for vacation pay after one year of employment.

o        Vacation pay is calculated by the average amount of hours worked per week for one year.

  • 401K

o        Heritage will match 100% of your 401(K) contribution for up to 3% of your base wages and will match 50% of an additional 2% of your base wages.

o        Employee must have completed 1 year of service to participate and have worked 1,000 hours during that year.

o        You must remain employed with Heritage Health Care and work 1,000 hours or greater each calendar year to remain eligible to participate.

  • Attendance Bonus

o        HHA’s are eligible for a bonus every month when they work the required number of hours.

o         Must be present for supervisory visits, and must not have any call-offs, cancellations, no-shows, or late paperwork to be eligible.

o        The bonus is calculated monthly and is based on the cancellation and payroll reports. 

o        $50 is paid for working 130 or more hours, $30 is paid for working 100 or more hours, and $15 is paid for working 50 hours or more per month. 

  • Referral Bonuses

o        Earn a $50 bonus when you refer a HHA to work at Heritage.

o        To be eligible, the referred HHA must work a successful 90 days and at least 120 hours as well as receive a good evaluation in order for the bonus to be received.
