The Home Infusion Therapy Program allows referral sources to place one telephone call and be assured that all aspects of care will be coordinated by qualified professionals in an efficient, cost effective manner.

Services May Include:

• Insurance investigation and verification
• Patient case conference with discharge planner, physician, agency representative and family
• Implementation of a pre-discharge, individualized patient teaching plan
• Evaluation of patient's readiness for successful discharge
• Acquisition and delivery of equipment, supplies and pharmaceuticals
• Admission seven days a week
• Extended hours program availability
